Position Title: Advance Care Planning (ACP) Facilitator
Reports To: Director of Operations

Position Summary
The Advance Care Planning (ACP) Facilitator plays a crucial role in guiding individuals and families through the process of understanding and documenting their healthcare preferences.
This position involves conducting educational sessions, facilitating sensitive conversations, and ensuring that patients' values, goals, and preferences for future care are clearly communicated and documented in advance care directives.
Key Responsibilities

Patient and Family Engagement:

Facilitate one-on-one or group discussions about advance care planning, healthcare goals, and end-of-life decisions.

Provide education on medical treatments, advance directives, and the importance of healthcare proxies.

Documentation Support:

Assist patients in completing advance care directives.

Ensure complete form as per organizational protocol.

Community Education and Outreach:

Conduct workshops, seminars, and presentations to educate the public about advance care planning.

Develop and distribute informational materials to raise awareness of ACP.

Compliance and Best Practices:

Stay updated on state and federal laws regarding advance care planning.

Adhere to organizational policies, confidentiality standards, and ethical practices.

Qualifications

Education:
-
Bachelor’s degree in social work, nursing, public health, or a related field (preferred).

Certification in advance care planning facilitation (Certification within 3 months of onboarding).

Experience:

2+ years of experience in healthcare, social services, or counseling (preferred).

Familiarity with advance directives, medical terminology, and end-of-life care (preferred).

Skills and Competencies:

Strong communication and active listening skills.

Empathy, cultural sensitivity, and the ability to navigate emotionally charged conversations.

Organizational skills and attention to detail.

Proficiency or willingness to learn various electronic platforms, i.e. Virtual Conferencing.
